First, call NVA member services at 1-800-672-7723 to check that your plan includes out-of-network benefits. Make sure you have your policy number handy when you call!
If your plan does allow for out-of-network coverage, great! Once you receive your receipt from us, you’ll need to send it to NVA to submit a claim.
To submit a claim by email:
- Print and fill part A of their out-of-network reimbursement form
- Send the form and include the receipt we emailed to you:
visionclaims@e-nva.com
To submit a claim by mail:
- Print and fill part A of their out-of-network reimbursement form
- After completing the claim form, you can print and mail copies of your claim form and your Simple Contacts receipt to:
