How to Submit a Claim to Davis Vision for Simple Contacts

The following is a step-by-step guide on how to submit an insurance claim to Davis Vision for reimbursement of your contact lenses

First, call Davis Vision member services at 1-800-999-543 to check that your plan includes out-of-network benefits. Make sure you have your policy number handy when you call!

If your plan does allow for out-of-network coverage, great! Once you receive your receipt from us, you’ll need to send it to Davis Vision to submit a claim.

To submit a claim online:

  1. Log in to your account
  2. Navigate to "Access Benefits and Forms"
  3. Download the Direct Reimbursement Claim Form
  4. Follow the instructions on the form to submit your claim (Note: You will include the receipt we emailed you here).

To submit a claim using the mobile app:

  1. Log in to your account
  2. Fill in all relevant expenses
  3. Take a photo of your receipt
  4. Once complete, you can track the progress of your out-of-network claim in the app!
