First, call Aetna member services to check that your plan includes out-of-network benefits. Call the Member Services number on your ID card for personalized service, or log in to your secure account at www.aetnavision.com and choose Contact Us to get phone number options.
If your plan does allow for out-of-network coverage, great! Once you receive your receipt from us, you’ll need to send it to Aetna to submit a claim.
To submit a claim by email:
- Print and fill out the Aetna out-of-network reimbursement form
- Send the form and include the receipt we emailed to you: email@example.com
To submit a claim by mail:
- Print and fill out their out-of-network reimbursement form
- After completing the claim form, you can print and mail copies of your claim form and your Simple Contacts receipt to:
